Introduction

Ontario Health holds itself to the highest ethical standards. Ontario Health employees are public servants and are subject to the conflict-of-interest rules and ethical standards for public servants under the Public Service of Ontario Act, 2006 (PSOA). This reporting platform that is hosted by an independent third party provides an anonymous means to report ethical wrongdoing involving current and former staff of Ontario Health, which includes fraud and/or “wrongdoing” as defined in the PSOA.

Wrongdoing under the PSOA includes conduct by individuals employed by Ontario Health that:

Ontario Health will review all reports received.

Instructions

To create a report, simply press the "Continue" button on the log-in screen. The system will provide you with a User Name and you will be asked to create a Password for the purpose of updating information to your file. The program includes a mechanism for you to upload any documentation or evidence. For security purposes, there is a sixty minute inactivity time limit built into this web application.
